We see you as a person, not just a case
Our discharge management team helps you organise follow-up care after you are discharged from hospital. We assist with applications for care levels and follow-up treatment (rehabilitation), help you find residential or outpatient care, and arrange for the provision of necessary assistive devices.
In addition, we offer psychosocial and social welfare advice during your hospital stay, particularly for patients at the cancer centres and their relatives.
Special Case: Geriatrics
The Clinic for Acute Geriatrics and Early Geriatric Rehabilitation has its own staff who carry out social services and discharge management duties there. Therefore, if you or a relative are a patient in our geriatrics department, please contact the staff at this department. You can find their contact details on the clinic’s ‘Team and Contact’ page.
Our responsibilities and services
- As part of our discharge management service, we work with you to plan and organise care tailored to your needs for the time after your discharge.
- We advise you on continuing care at home, including all necessary arrangements, as well as on inpatient care in the form of short-term or long-term care. We assist you with the application process and the necessary arrangements.
- Discharge management is initiated by the treating doctors, the nursing team and other healthcare professionals at the hospital, or you or your relatives may contact us directly.
- We advise you on upcoming medical rehabilitation measures and handle the associated cost arrangements.
- In connection with an illness and the resulting hospital stay, questions and problems may arise in your personal, professional and financial life, and we are happy to help you resolve them.
- We complement the medical and nursing care and work closely with all professional groups within the hospital.
- We provide you and your relatives with personalised advice through one-to-one consultations and put you in touch with the necessary support services and contact persons.
- Where necessary, we take charge of planning and coordinating the various support services and assist you with the application process.
- In addition, we offer advice on social welfare law issues.

We need your consent in order to act on your behalf.
As part of the discharge management process, it may be necessary for the hospital to contact health insurance, long-term care insurance or service providers. In such cases, it may be necessary to share your patient data with these parties for this purpose. We may only pass on your data with your written consent. By signing this, you confirm that you agree to the discharge management team acting on your behalf and that they may pass on the necessary data to your health insurance provider, long-term care insurance provider, support services, etc.
If, as a patient, you do not wish to have discharge management arranged or do not want your health insurance or long-term care insurance to be involved, we will of course respect that. In that case, simply do not give us your consent.
However, this may mean that follow-up measures are not initiated or do not begin in good time. Similarly, without our discharge management, you may find that you are delayed in submitting your applications for benefits.
You may withdraw any consent you have already given to the Discharge Management team in writing at any time. Similarly, even if you initially declined, you may still agree at a later stage during your inpatient stay to involve our Discharge Management team.

How to find us
The Discharge Management and Social Services offices are located on the first floor in the passageway between the main building and the medical centre.
When you enter the foyer through the main entrance and turn sharply left at the information desk, a corridor leads straight ahead to the medical centre. In this corridor, you will find the entrance to the Discharge Management and Social Services office block on the right-hand side.
If you enter the buildings via the entrance to the Medical Centre (next to Engelbrecht Bakery), please go straight ahead up the small flight of stairs, past Bistro 54°, into the corridor leading to the main building. In this corridor, you will find the entrance to the Discharge Management and Social Services office block on the left-hand side, just past the glass door.
